NH Care Connections Assistance Request Form is LIVE! Expand Your Capacity and Connect Clients to Support

Granite United Way is excited to announce the launch of the NH Care Connections Assistance Request Form, a new, user-friendly resource available now!

This form is specifically designed to help an organization manage clients when they do not have the internal capacity to serve them directly.
How It Works:
1.      Client Submits Form: A client fills out the NH Care Connections Assistance Request Form.
2.      Live Connection: The client is connected to a Connect Navigator who can assess their needs and link them to appropriate resources across New Hampshire.

Important Distinction: This is NOT a Crisis Service
This form is for general social service assistance. For emergencies or mental health crises, please ensure clients utilize dedicated services: 911 or the 988 Suicide & Crisis Lifeline.
